Families First is a referral based organisation. We work with children and families who have been referred to us through partner agencies such as, health, education, and social work. Please find our child and adult referral forms below.

We ask that you do not worry too much if you cannot answer all the questions, or that they are not relevant, as long as you have consent from the person to say they are happy to be referred. The more information we have the better we can support the person you are referring.
